You can rest assured that we, as well as the doctors and midwives at the hospital, will do our best to explain to you right then and there what might be going on and what you can generally expect. However, there are two situations we\u2019d like to briefly explain now: a birth with the assistance of a <strong>cesarean section<\/strong> and a birth with the help of a <strong>vacuum pump<\/strong>.<\/p>\n<p><strong>In the Netherlands, 17 out of every 100 babies are born via cesarean section<\/strong>: In half of the cases, this was planned, and in the other half, the decision was made during labor. If it is decided during your pregnancy that your baby must be delivered by cesarean section, we call this a <strong>scheduled cesarean section.<\/strong> It\u2019s also possible that you\u2019re in labor and that, during delivery, it\u2019s decided that your baby should be born via C-section after all, because this is better for your health or the baby\u2019s health. We call this a <strong>emergency cesarean section<\/strong>. The following situations may warrant a cesarean section: your cervix is not dilating (even after you may have been given medication to strengthen your contractions); you are not making progress with pushing (and the baby isn\u2019t far enough down in your pelvis to be delivered with the help of a vacuum extractor); the baby\u2019s condition is deteriorating, and the doctors are concerned that waiting any longer for labor to progress or for you to push might take too long for the baby and could cause harm.<\/p>\n<p>The decision to perform a cesarean section is made by an obstetrician-gynecologist or a resident in obstetrics and gynecology. This doctor will also perform your cesarean section. Once the decision to have a cesarean section has been made, you will be given a surgical gown to wear, and a<br \/>\n<strong> IV<\/strong> get a shot and you'll get a <strong>catheter<\/strong> inserted to empty your bladder. The operating room team will be notified, and as soon as you are fully prepared, the <strong>operating room<\/strong> Once everything is ready and everyone is prepared, you\u2019ll be taken\u2014on your bed\u2014to the operating room wing. At the Meander Medical Center, this is on the same floor as the delivery rooms. Your partner will be given a special gown to put on over his clothes and will accompany you to the operating room. If you haven\u2019t yet <strong>epidural<\/strong> If you haven\u2019t received it yet, you\u2019ll get it as soon as you\u2019re in the operating room. In addition, the medical staff in the operating room will explain as clearly as possible what they\u2019re going to do. The time until your baby is born is very short; often, your baby is born within about 15 minutes after you\u2019ve been wheeled into the operating room. The doctor will show you your baby, and your baby will be taken briefly to another room where the pediatrician will examine them; your partner will be present during this time. If your baby is doing well and the situation allows, the baby will be brought back to you in the operating room. This isn\u2019t always possible: the operating room is relatively cold, so a nurse must be available to keep a close eye on your baby. If this isn\u2019t possible, and provided the baby is doing well, your baby will be placed on your partner\u2019s chest so the baby can have skin-to-skin contact. This doesn\u2019t happen in the operating room, but almost always in the \u2018delivery room\u2019 where you came from. After your baby is born, it takes about another 30 minutes for the cesarean section to be fully completed: the placenta is removed from your uterus, blood loss is monitored, and the incisions in your uterus and abdomen are sutured. After the cesarean section, you\u2019ll go to the recovery room, where\u2014as soon as you\u2019re feeling well enough and the baby\u2019s health allows it\u2014the baby can be brought to you as soon as possible. A cesarean section is a major surgery, so you\u2019ll be given pain medication afterward. After a cesarean section, you and your baby will stay in the hospital for at least 24 hours before you\u2019re allowed to go home.<\/p>\n<p>You may be in labor and already quite far along: you\u2019re already pushing! Even then, there may sometimes be a reason to seek extra help\u2014for example, because your contractions are slowing down, the baby is in a more difficult position, or because you\u2019re extremely tired. It\u2019s also possible that the baby\u2019s heart rate reacts so strongly during pushing that there are concerns about the baby\u2019s well-being or health. A vacuum extractor can then help the baby be born more quickly\u2014for example, in 2 contractions instead of 10\u2014so that your baby has the energy needed for the transition from inside your womb to outside. A <strong>vacuum pump<\/strong> can only be used if the baby is already very far along; the baby must have already passed the bend in your birth canal. As preparation, a <strong>cut<\/strong> first, your pelvic floor is numbed. A vacuum pump is a metal cup that is placed over the baby\u2019s head, and a vacuum is created inside it to provide \u2018grip\u2019 on the baby\u2019s head. A vacuum extractor doesn\u2019t literally pull the baby out: what the doctor does with the vacuum is apply pressure when you push, so that the baby doesn\u2019t slip back after the pushing contraction but stays in the same position. This way, during a pushing contraction, it\u2019s not three steps forward and two steps back, but only three steps forward. If the decision is made to use the vacuum extractor, the expectation is that the baby will be born after about one or two contractions. Very rarely, the baby is not born after the vacuum extractor is used, in which case a cesarean section is still necessary. A baby may experience a headache after a birth with the vacuum extractor. The hospital staff will explain this to you; in many cases, your baby will be given acetaminophen for this after birth.
